We will be accepting applications
from all interested persons but priority will be given to those at or below the
250% poverty threshold ($27, 075 yearly).
Applicants must provide previous
year’s tax return and the financial worksheet attached to application.
2. Professional/Medical documentation.
All applicants must be 18 years or
older and provide the following documents:
A. Letter
from a therapist who follows WPATH Standards of Care
B. Letter
from primary care provider or surgeon stating that applicant is in good health
and qualifies for surgery.
3. Must be a Florida
resident.
4. Must use a Florida
surgeon.
5. Must meet volunteer requirements.
Applicant, if chosen, must
complete 40 hours of volunteer service related to GLBT issues (preferably
transgender issues). If applicant resides in the Tampa Bay
area, hours will be completed under Transgender FORGE’s supervision. If outside
of Tampa Bay, FORGE board of directors must
approve organization applicant wishes to volunteer with.
6. Essay
Applicants will be required to
submit a 2-5 page essay explaining why they are applying to the FTM Fund, how
getting surgery will benefit their life and how they intend to make an impact
in the future on the transgender community. Applicants should feel free to
relate their experiences both negative and positive in their personal lives, in
the workplace and in school.
Do not be intimidated by the
essay. Applicants will not be judged based on grammar or spelling.
7. Interview
Applicants will be required to
attend a personal interview with the selection committee during the month of
May. Applicants will be contacted with a date.
The selection process will be
“blind”, meaning applications will be reviewed by the selection committee after
each applicant is assigned a number. The selection committee will not know the
name of any applicant until the personal interview.
The number of grants given each year will be
dependent on the funds available and whether the applicants are able to contribute
to their surgery. FTM Fund will offer grants no larger than $5000 per person
based on applicant’s ability to contribute and the cost of surgeon chosen.
We suggest that during the
processing period and even prior to having surgery, if you are selected, an
applicant continue to save money toward surgery. We will be offering partial
grants as well, when funds allow.
A “back-up” applicant will be
selected in the event that the original applicant cannot meet the requirements.

Applicants are responsible for
their own transportation costs to and from personal interview and surgeon
visits.
